Travel Advice

If you are due to be travelling abroad we advise you to complete a Travel Questionnaire as soon as possible prior to your departure, but please allow at least 6 weeks as some vaccines require completion of a course and some are required to be ordered in. These can be collected from the Reception or downloaded from the link below.

Travel Questionnaire

The form should be completed and left at the Reception Desk. Our Practice Nurse will look through your form and assess what advice should be given, whether any holiday vaccinations are needed and if any anti-malarial advice and/or medication is appropriate for your trip.

Available Vaccines:

  • Hepatitis A
  • Typhoid
  • Tetanus
  • Diptheria
  • Polio
  • Cholera

Anti-Malarial Medication

If Chloroquine or Proguanilare recommended these can be bought at the Pharmacy. If Doxycycline, Malarone or Mefloquie are recommended an appointment with the doctor is needed.
