Access to Medical Records

How do I access my medical records?

Your medical records include information about your medication, allergies, vaccinations, previous illnesses and test results, hospital discharge summaries, appointment letters and referral letters.

You can access your GP records through GP online services.

If you wish to utilise this service you must register IN PERSON at Reception where you will be provided with a username and password. For more information visit https://www.nhs.uk/NHSEngland/AboutNHSservices/doctors/Pages/gp-online-services.aspx

For Data Protection purposes, valid photographic ID will be required before this service can be activated. Once registered you will be provided with a printed letter detailing how to acess this service.
